Please include a description of the donation and note if there is an expiration date or restrictions. * For wine donations, THS will provide donors with a copy of the ABC License prior to the event. Please include in your description the vintage, winery, name of the wine, container size, and quantity. * If the donation is a gift certificate, please note if you will be providing one or if you would like THS to generate a certificate. * Please list any restrictions or limitations (blackout dates, expiration, capacity, etc).

Estimated Value of Donation

The Healdsburg School is a 501(c)(3) organization; we will send documentation of the donation for the donor's tax purposes. If the donation does not have a ready retail value or donation documentation is not needed, feel free to write "N/A" or "contact me for more details."
